Do you have a Studio Director Account Set-Up with a valid credit card on file? If no, create an account prior to completing this form
Yes, and I have a valid payment method on my account

Create a Studio Director account following this link: https://app.thestudiodirector.com/thefloordanceacademy/portal.sd?page=Login


Add a payment method in Studio Director following these steps:

1) Click on the 3 horizontal lines in the top left corner

2) Click on "edit payment method on file"

3) Add name, address, email address, then click NEXT

4) Enter credit card details then click OK

5) You will then see your card details but it won't be added to your account until you click SAVE

MONDAY SCHEDULE (Tuition will be charged for all boxes checked $40/class)

Choose the Thursday classes you are interested in trialing:

Choose the Friday classes you are interested in trialing:
